The Trans person wishes to have, for the most part, the secondary sexual characteristics that are attributed to their lived and felt identity from the age of 4, 5 or 6 years old. Gender Variants (transsexual, transgender, gender nonconforming people) are not mental illnesses. Not all transsexual adolescents will need endocrinological intervention, nor will they all need the same level of intervention. Associated masking processes must be excluded: Disorders of Sex Development (DSD), Gonadal Dysgenesis, True Hermaphroditism (ovotesticular DSD, Male Pseudohermaphroditism (XY), Morris Syndrome, Swyer Syndrome, Female Pseudohermaphroditism (XX), Adrenal Hyperplasias with ambiguous genitalia.

Each child is unique, there is no single correct way of being and, for this reason, it is not possible to speak of universal formulas to educate sexuality. For example, at the age of 11, there are girls who are still playing with dolls while others are more concerned with seduction or love games; some still have the bodies of girls while others seem almost adult; some like to play sports while others prefer to sunbathe, etc. With the incorporation of girls and boys from other countries in schools and neighborhoods, this disparity is even greater. It is a pending issue that we health professionals have with trans people, who have the right to be informed and treated according to their specific needs and peculiarities. Respect and active listening, in my case, to their sexual identity must be, and has been since 1997, the integral approach to the sexuality of transsexual people.

At what age can these dissociative feelings appear?

They are vivid, real feelings from early childhood. These children are not affected by dissociation, (dissociation is psychosis, a serious mental illness), this does not occur in Transsexual people, simply their sexual identity is different from the sexual roles assigned to these people by their genitals but not by their felt sex, that from the age of 6 years, if we let them and listen to them, even before, they manifest I am a boy or I am a girl. They are not in the wrong bodies, nor do they have to experience the discomfort that comes from not being respected in their identity by the family, school and social environment. The concept of masculine and feminine in science are among the most confusing and must be broken down in at least three directions:

  • Sense of activity/passivity.
  • Biological sense (sperm/ova)
  • Sociological sense Observation
