Modern Blepharoplasty

Blepharoplasty is an operation of Aesthetic Plastic Surgery that aims to improve the functional and aesthetic appearance of the eyelids and the contour of the eyes. In these times of masks and social distance, the look is a fundamental tool of communication and empathy.

Traditional blepharoplasty – still practiced by many surgeons – consists of the removal of the upper and lower fat bags and a strip of excess eyelid skin. This achieves, correctly performed, acceptable results but does not restore the youthful appearance of the gaze.

Modern blepharoplasty is based on a more reconstructive and rational approach and less ablative (ablation is extirpation, removal, amputation). Modern blepharoplasty mobilizes, tightens and replenishes the tissues so that the eye contour regains its youthful appearance and avoids most of the complications of classic blepharoplasty.

Most common complications of blepharoplasty

  • Round eye and retracted lower eyelid that is avoided with the suspension of the lower orbicularis oculi muscle and minimal skin excision.
  • Sunken eye caused by the removal of the fat pads, which is avoided by repositioning them on the rim of the orbit which provides a short and youthful lower eyelid.

Naturally, all of the above has its exceptions that must be considered in a careful evaluation of each particular case carried out in the visit prior to any surgery.

Logically, blepharoplasty can be performed simultaneously with other facial procedures. With anti-aging techniques (lifting, thread tightening) or other surgeries such as rhinoplasty.

In my experience, the simultaneous use of laser resurfacing and chemical peeling on the eyelid skin with blepharoplasty should be avoided, as they can cause severe eyelid retraction.

As in any surgical procedure there is a possible risk of complications, which are remote if the technique is performed carefully, and should be exposed to the patient before surgery and thoroughly reflected in an informed consent.

The postoperative period of blepharoplasty is very bearable. Cold should be applied in the first hours and appropriate medication. It is important a close control and easy communication between patient and surgeon. WhatsApp is very useful in our case.

Once the stitches are removed, after 5/7 days, the patient can prudently return to normal life. And always under the control and zealous supervision of the Plastic Surgeon.
