This is probably one of the most frequent questions asked by patients in the Plastic, Aesthetic and Reconstructive Surgery office regarding breast augmentation. That is to say, their question is: how do I get a result that is not noticeable after surgery?
Hiding the scar, the first point to consider so that a breast augmentation looks natural
The first point that makes a breast look operated is the scar. That is why, whenever possible, in most breast augmentations, even in tuberous breasts, it is necessary to avoid leaving the scar visible on the breast, both in the areola and in the groove. This is achieved by making a scar in the highest fold of the armpit, so that the scar is completely hidden in one of these folds. This means that the breast no longer has a surgical scar.
The shape of the breast influences the naturalness: implant used, projection and volume.
The second point is about the shape. Here it depends on what the patient wants. That is, depending on the shape of the implant that is placed and, depending on whether the patient is more or less thin, or has more or less breast before surgery, it will notice the shape of a rounder breast (more breast augmentation). Or if the patient wants a more discreet breast and not be noticed operated, more anatomical implants will be used, especially in patients who have very little chest. How will a completely natural feel be achieved in a breast augmentation?
The third point is the touch, that is to say, how the prosthesis feels to the touch and what the patient feels if someone touches the breast. If a patient already has enough breast or has a little breast before the surgery, if she does not have a very big implant, the touch will be the same, maybe a little firmer but very similar to the one she had before the surgery.
When is the touch altered and when, after an operation, will it be noticed that it is a harder implant, with a prosthesis touch? It happens in patients who did not have any breast before surgery and in very thin patients, where it can become more evident that the patient has an implant although no scars are visible. Lipofilling, that is, fat grafting, is used to avoid this. This allows the creation of a lipovet (as if it were a coating of the patient’s own fat) around the implant, which will allow the breast to have a completely natural feel, that is, the feel of fat and not of the implant.

Another important point, not only for naturalness but also for the patient to be satisfied with the implant, is the separation between the breasts or the distance between the breasts. After placing the implants there were patients who, because of their anatomy, the cleavage was very separated.
