Treatment against hemorrhoids: the mechanical circular mucosectomy of longo

Hemorrhoids, or more commonly known by the term “piles”, are venous plexuses that exist in the anal canal below the mucosa and are physiologically present in all people. When hemorrhoidal pathology or disease occurs, there is a dilatation of these plexuses with the formation of pathological hemorrhoids, which can lead to prolapse outside the anal canal (the descent of the rectum).

What are the symptoms of hemorrhoids?

The patient suffering from this condition feels pain and a tingling or irritation of the skin, in addition to its main symptom: spontaneous palpable hemorrhoids on the outside, generating bleeding, especially after defecation. The latter is the most alarming symptom for patients, because it is so striking.

Causes and treatment of hemorrhoids

Although it is true that a causal relationship has not been established, hemorrhoidal pathology is usually associated with constipation and pregnancy. Depending on the physical examination performed by the specialist in proctology or the symptoms produced by the patient, different degrees of hemorrhoids are established, for which there are treatments of choice to be established by the specialist individually.

Some of the techniques to treat the condition are:

– Hygienic-dietary measures, such as sitz baths or trying to avoid constipation, among others.
– Rubber band ligation: A simple technique that consists in the application of a rubber band over the hemorrhoidal bundle in order to cut off the blood flow in the area and the subsequent drying of the hemorrhoid.
– Classic hemorrhoidectomies: Surgical removal of both internal and external hemorrhoidal veins. It requires a postoperative period and a slow recovery, but it is one of the most effective.
– Doppler-guided arterial ligation (LAHGD): Consists in the elimination of the hemorrhoidal bundles, decreasing their volume until the symptoms disappear permanently.
– Mechanical circular mucosectomy or Longo’s technique: Consists in the removal of hemorrhoid tissue, based on the ascent of the mucosa of the anal canal, at the same time as the vascular inflow to the hemorrhoids. It is a method that offers a decrease in postoperative pain in which Dr. Costa has accumulated extensive experience.

Of all these techniques, the ones that have proven to be most effective are the classic techniques, although when there is a correct indication and in a personalized way for each patient, the other techniques can be applied with good results and certain advantages.
